I think that rather than having FT and regeneration disabled in the menus (which would still be very easy to turn off) give the player an option during character creation to have a 'hardcoe mode' which offers the above challenges.

Shouldn't this be in the Official: Beyond Skyrim thread?

Morrowind wasn't unleveled. For example, I don't remember Nix-hounds showing up near Seyda Neen until you get a few levels into the game. I've never seen them there at level one.
