About this list:
The aim of this mod list is to provide a list of, what I feel, to be good gameplay and immersion mods that will add to the challenge and diversity of the Fallout 3 experience. There were a lot of these lists around for Oblivion to help new mod users see what was out there, and there are very few for Fallout. As the author of Fallout 3 Wanderers Edition (FWE) I always keep an eye out for mods that either go along with FWE nicely, conflict with it, or are part of FWE, so I will make note of that as well. Otherwise, mods are broken down by category. Often times mods within a categoy will conflict with one another, and I will try and make a note of this to the extent possible and provide a description to help point out significant differences. Here is a key to some of the additional tags for mods:
[FWE Companion] - If you are using FWE, I highly recommend using this mod along with it. These mods won't cause a serious conflict either.
[FWE Integrated] - This mod, a prior version of it, or something very similar has been built into FWE, so don't use it if using FWE.
[FWE Incompatible] - This mod will likely conflict with FWE, as FWE provides a similar feature, so don't use it if using FWE.
[Mez's Pick] - This mod is a personal favorite or recommendation within a mod category.
Bold Names Mods - Mods that are newer or more regularly updated.
Non-bold mods - Mods that may be older or less developed, but still worth mentioning.
In general, I do not want to include individual weapon or armor mods on this list, or pure model/texture replacer type mods. I'd like to keep the focus on gameplay and immersion, rather than graphic quality or new content. That being said, please feel free to mention mods I've missed or make a pitch for your personal recommendations! Mod Tools + "Standard Resource" Mods
There are a number of basic mod use tools that I encourage everyone to download, install, and learn how to use. This includes:
Fallout Mod Manager (FOMM) | FO3 Nexus
Essential modding tool. Use it to order your mod list correctly and launch your game. Includes a number of other helpful utilities, like Archive Invalidation, A .bsa extractor, "FOMOD" support, and a save game manager.
Fallout Script Extender (FOSE) | Homepage
A utility that adds a number of new functions to scripts. This is a required utility for many mods.
FO3Edit | FO3 Nexus
Plugin editor and conflict detector with a lot of helpful features. I use this more than the GECK for making and viewing plugins.
Cipscis' Automatic Save Manager (CASM) | FO3 Nexus
Provides a newer, safer, and mbetter save game system, replacing the buggy, glitchy, less-than-effective autosave system that can corrupt your save files.
CALIBR - Community Ammunition Library | FO3 Nexus
This is a small mod that adds new custom ammo types to the game. It is a standard mod resource, and many mods will require CALIBR as a master. Required for FWE.
CRAFT | FO3 Nexus
Another mod resource that allows multiple custom workbench formulas to work at a time. Required for FWE.
Cornerstone Mods
A number of large "overhaul" type mods and others have been developed that greatly expand gameplay in one more areas. These mods often form the cornerstone of people's load order or are a staple "must have mod." Many of the mods listed here are also part of the FOIP program (see first entry), and as such extra effort has been made to provide cross compatibility.
Fallout Interoperability Program (FOIP) | FO3 Nexus
Maybe you've heard of FCOM for oblivion? This is currently the closest thing to FCOM for Fallout. A number of "large mod" authors are using "compatibility patches" organized through FOIP to provide cross-compatibility between many of the more popular big mods. Presently, this includes Mart's Mutant Mod, Fallout Wanderers Edition (FWE), Fallout Overhaul Kit (FOOK), Weapon Mod Kits (WMK), Energy Visuals Enhanced (EVE), as well as XFO, and Classic Fallout Weapons.
FWE- Fallout Wanderers Edition | FO3 Nexus
Fallout 3 "Wanderers Edition" large overhaul mod that integrates many other mods in conjunction with developing its own content to provide a comprehensive reworking of the wasteland and the player experience. Wanderer's edition aims to improve the the challenge, sense of immersion, and depth of gameplay compared the vanilla game. The "Wanderers Edition" emphasizes balance, choices and consequences, role-playing, and (to the extent that gameplay remains fun and exciting) "realism." Includes weapon and armor rebalance, skill/SPECIAL/leveling changes, new combat features, new food a drinks, and daily "needs" mod, and much more.
Martigen's Mutant Mod (MMM) | FO3 Nexus | [FWE Companion] | [Mez's Pick]
A hugely popular mod that expands the diversity of creatures and NPC spawns. New skins, entirely new creatures, and a number of optional modules makes MMM a must-have mod for improving Fallout. The increased spawns optional module is worth it alone, making fights bigger and more dangerous.
Weapon Mod Kits | FO3 Nexus | [FWE Companion] | [Mez's Pick]
This is a brilliant mod that adds the ability to attach "mod kits" to your weapons, including silencers, scopes, extended magazines, laser sights, and auto-fire kits. The mod kits can be purchased or found as loot throughout the wasteland.
Fallout Overhaul Kit (FOOK) | FO3 Nexus | FookUnity
This is another widely popular mod, that adds TONS of new weapons, armor, and other goodies to the wasteland, revamping loot lists and factions in the process. A number of other gameplay improvements and features are also included that will provide an entirely new gameplay experience. Generally speaking, FWE and FOOK touch many of the same things, and have a different approach to balancing. However, Offkorn's / Streetstar5's FWE-FOOK Merger provides compatibility patches to get these two mods playing together for those of you interested in running both!
Energy Visuals Enhanced (EVE) | FO3 Nexus
EVE greatly improves the visual and graphic effects of energy weapons, adding more diversity, flair, and excitement to the weapons.
Darn's UI | Darn's Page | [Mez's Pick]
Absolutely essential user interface (UI) mod. Take the console-itis out of fallout. Creates more text space on menu's throughout the game, and just makes everything feel like a real PC game. Really important!
Unofficial Fallout3 Patch | FO3 Nexus
Pretty popular mod that fixes or tweaks a lot of small glitches, oversights, and minor bugs in the original game.
Comprehensive Gameplay Rebalance Mods
These mods all address a range of gameplay topics, such as combat, VATS, drugs and chems, item rarity, health and level advancement, skills, perks, enemy balance, weapon/armor balance etc.... If is advisable to understand what these mods do, and make sure when using them what other mods might create compatibility issues.
Arwen's Realism Tweaks | FO3 Nexus | [FWE Integrated - Partial] | [Mez's Pick]
An excellent set of gameplay rebalance tweaks if you're looking for a streamlined rebalance of the existing game. Arwen's tweak includes combat and skill changes, improved AI and sneak settings, weapon and armor rebalance, and a number of "survival" oriented changes. Very concise and to-the-point changes. This mod is also fairly modular. FWE includes Arwen's Smarter AI and sneak changes.
Fallout 3 Balance Overhaul (XFO) | FO3 Nexus | XFO Home Page | [FWE Integrated - Partial]
Very comprehensive overhaul mod. XFO is highly modular and contains a number of alternative setting packages that really allows the user to tweak the mod to their own liking. However, the mod is no longer under active development.
Fallout Advanced | FO3 Nexus | [FWE Incompatible]
The Castle's mod is a newer gameplay rebalances mod that includes a number of interesting and novel changes to the core gameplay mechanics. Definitely worth a look at.
Mini Balance | FO3 Nexus | [FWE Incompatible]
Fairly comprehensive rebalancing mod for most the major gameplay areas, including equipment balance, combat damage + accuracy settings, VATS tweaks, healthing, stealth settings, power armor and more. Choose your poison!
BRUTE | FO3 Nexus | [FWE Incompatible]
Another newer gameplay overhaul that focuses mostly on combat and weapon/armor rebalancing. A few new gameplay features are included as well to keep things interesting.

S.P.E.C.I.A.L.s / Skills / Leveling / Experience

Adjusts the starting skill values, and ties skill increases into attribute increases. Modified the amount of skill points gained per level, adjusted the effect of tagged skills, etc. I think this is a great mod that makes SPECIAL more important.
Wasteland Mastery | FO3 Nexus
For all you power gamers out there, this mod will let you raise your skills beyond 100. Skills raised above 100 provide special bonuses. However, for this mod to work you will need to use the Level Cap Workaround AND the Level Cap Skill Increaser Workaround.
Slower Leveling | FO3 Nexus | [FWE Incompatible]
This mod provides a number of different modules to control how quickly you level up - mostly slowing the speed of level ups down. In addition it adjusts the XP rewards for better balance.
Raygun Classic | FO3 Nexus | [FWE Incompatible]
Changes many stat related to formulas (health, carry limit, action points, skill gains, etc.) to make things work a bit more like prior Fallout games.

Adjusts skill gains, similarly as Pointiac. Tag skills provide +2 to skills per point spent, and the number of skill points received per level are reduced.

Adjust the player's hitpoint formula to reduce the number of hitpoints but make strength + endurance more important.
Perks

Adds a three-step perk that allows you to repair and control busted robots. Yay for smart people!

Similar to the above mod, but for fuzzy creatures. Series of perks let you tame stronger and stronger animals so they become your precious followers.
Buildable Bots | FO3 Nexus
This mod lets you collect robot parts and take them to a new facility and build your own robotic minions. Great fun. The Robco Certified Buildable Bots patch lets you use this mod alongside Robco Certified.
Ultimate Perk Pack | FO3 Nexus
This mod adds a LARGE number of additional perks as well as optional rebalancing of the original game perks. A very popular mod that adds a lot of diversity to the perk lineup. The new perks will work with FWE, although you should NOT use the original perks rebalanced part of this mod.
Implants | FO3 Nexus
Adds "implants" you get from doctors, which are really just perks that you buy. The implants generally are used to raise 1 attribute, but the downside is that it lowers another. Pretty cool idea that could be developed even more.
Books!

This is a great mod that removes the skill point rewards for reading books . . . BUT after reading a few copies of each book you INSTEAD gain a level for a new perk that provides something fun and exciting.
Persistent Skill Books | FO3 Nexus | [FWE Integrated]
Whoever decided that it made sense for books to disappear after you read them? Well this mod knocks some common sense back into things. After reading a book, it will appear in the miscellaneous section of your pipboy next time you open it. Incompatible with the Book Perk mod above.
Pre-War Books + Titles | FO3 Nexus | [Mez's Pick]
Another book perk related mod, which IS compatible with the mod listed above. This mod randomly replaces "pre-war" books randomly with 1 of 150 actual book titles (with covers too!) as well as occasionally replacing "ruined" books with a named pre-war book. New perks are given to you for either reading or destroying these books. Are you naughty or nice?
Hacking + Lockpicking
Menu Based Hacking + Lockpicking | FO3 Nexus | [FWE Integrated]
Formerly called skill based Hacking + Lockpicking, this mod provides the option to skip the mini-games and use a true RPG chance based system to determining success. The advantage is that there is now a chance to pick/hack harder locks than your skill would otherwise allow.
Explosive Entry | FO3 Nexus | [FWE Integrated]
This mod adds a new optional way to open locks - using explosives. You can adjust the timer and pick the explosive to use. The chance of success is based on your repair skill.
Zumbs Overhauled Real-Time Security | FO3 Nexus | [Mez's Pick]
Adds "real-time" options for picking and hacking, basing and blasting locks, and more.
Weight Capacity + Item Weight
Mighty Mouse Weight Mod | FO3 Nexus | [FWE Incompatible]
Includes a bunch of .esps that change the base weight limit and weight increase per strength. Should cover just about anyone's tastes.
Miscellaneous Items Fix | FO3 Nexus | [FWE Incompatible]
Adjusts weights of misc items to be a little more sensible.

Adjusts the weight of aid and miscellaneous items to be more realistic, similar as above except it includes aid items.
Karma

Makes higher levels of good or bad more difficult to achieve. The range of "neutral" karma is expanded a bit as well. Removes repeatable/lame ways of changing karma.

Removes the irritating Karma pop-up messages that occur all time, like when I steal Doc Hoff's drugs.
Item Repair / Crafting

More recent update of the original "repair rethought" mod that greatly expands the range of options for repairing items, adds cross-repair support for similar types of equipment. There is also an update patch for this mod to support some of the DLC.

Integrates XrepairWeapons and CrossRepairArmor with some extra additions into a single patch.
Haldur's Improved Workbench | FO3 Nexus | [FWE Integrated]
This mod does two things. First, when standing next to a workbench, your repair skill is increased by +10. Secondly, as you repair skill increases and you are next to a workbench you'll be able to use a greater variety of items to repair things. This mod is not compatible with the two listed above. The FWE integration includes only the repair skill bonus.

Movement + Stealth

An awesome mod for those that don't really like VATS. Adds an alternative VATS system (you can still use the normal VATS with this mod) that is just a slowed down version of "real time" combat. Pretty cool idea with a great implementation. Entering bullet time slowly drains your Action Points. Has an in-game settings menu.

Very cool mod that adds sprinting to the game for when you need that extra dash of speed. Sprinting will drain your action points, so be careful when you sprint! Also features a number of new sprint related perks, such as charging into and knocking over your foes. Has an in-game settings menu.
Speed Mod V2 | FO3 Nexus | [FWE Incompatible]
Mod that tweaks the various movement speed settings, and includes six variants. It works by increasing the base movement speed (i.e. walk) to be a little faster, and then tweaks the run multiplier in different ways.
Slower Backpedaling | FO3 Nexus | [FWE Integrated]
Who can run as quickly backwards as they can forward? Well you can't anymore with this mod. No more running backwards like a sissy shooting at melee super mutants who will get to you.
Realistic Sneak Mod | FO3 Nexus | [FWE Incompatible]
Removes sneak messages (hidden / detected / caution / danger) from the HUD. Makes sneaking around a lot more challenging. You need to listen to your enemies and watch your radar more carefully to tell if they are onto you or now.
Stealth Kills Enabled | FO3 Nexus
This mod changes the way that detection events work, so that you can now escape crime detection for killing provided that no one witnesses your devilish deeds.

Adds a system for disguising yourself by wearing other faction armor types. Chance based sytem includes a formula for being detected. Claims to work with all other mods, but haven't tested it yet. sounds interesting!

Adds a configurable hotkey that lets you quickly throw grenades and switch back to your prior weapon. Similar to grenade hotkeys in other FPS games.

Modular mod that's lets you pick and choose the degree to which skills, weapon condition, etc. effects your real-time accuracy.

This is an excellent mod that rebalances nearly every weapon in the game as well as adding unique meshes, textures, and and effects to unique weapons. The balance is very good, and gives a purpose to most weapons.

Increases the blast radius for explosions. A few different flavors to try.
Explosive Knockdown | FO3 Nexus | [FWE Incompatible]
Adds a knockdown effect for actors caught in the explosion radius. Incompatible with the realistic explosions mod listed above.
Explosive Explosives | FO3 Nexus | [FWE Incompatible]
Explosives, like mini-nukes, missiles, and grenades, now explode when shot directly. What a fun idea!
A Call of Duty Realism Mod | FO3 Nexus | [FWE Incompatible]
Appears to change primarily weapon stats in order to create a more realistic and brutal combat experience.
Super Sledge Knockdown | FO3 Nexus | [FWE Incompatible]
Adds a sweet knock back effect to foe's or the PC hit by the super sledge.
Vats

Players take 100% damage in Vats. The MFO combat overhaul covers this.
Simple Action Point Recharge Mod | FO3 Nexus | [FWE Incompatible]
Provides a number of different options for controlling how fast action points recharge.
Fast VATS | FO3 Nexus
Reduces or eliminates the difference in time between the player and enemies. This mod will make VATS less forgiving, that is, you're more likely to be killed a miss your shots since the targets are moving at the same speed relative to the player. FWE includes optional modules based on this mod.
Fast but Dramatic VATS | FO3 Nexus
Similar to the above mod. In general, animations are in realtime, but certain special hit animations still slow down for that dramatic appeal. FWE includes optional modules based on this mod.

Adjusts game settings to better balance the VATS accuracy formula, and slightly alters non-VATS accuracy as well.
VATS Normal Weapon Degradation | FO3 Nexus | [FWE Incompatible]
Adjusts a game setting so that weapons don't degrade extra fast when firing in VATS.
Auto-aim Related

Radiation

This mod changes the effects caused by each level of radiation sickness, in general to be much more severe. At really high radiation levels, you're character will actually start to glow. Oh no!

Increases the rate that you accumulate rads when wading or swimming in water.
Injuries + Healing
Triage | FO3 Nexus | [FWE Integrated] | [Mez's Pick]
This is a great mod that balances the desire for more realistic injury treatment without adding too much complexity or headache to the process. Essentially, in order to heal broken limbs or body parts, you need to use medical braces or surgical supplies (a replacement for the surgical tubing item) to fix yourself up. The mod adds some new doctor related perks that let you heal your limbs to a greater amount and recover used braces or supplies. One of my favorites!

This mod is slightly more complex than Triage, adding a number of new medical supplies and methods of healing yourself, for those who are looking for a deeper healing system.
Amplified Crippled Effects (ACE) | FO3 Nexus | [FWE Incompatible]]
Increases the effects of getting crippled . . . i.e. slower walking, more serious accuracy penalties, a "fade-to-black" concussion effect, and more. Very popular mod!
Weapon Restriction While Crippled | FO3 Nexus
Throw back to fallout 1/2. When 1 arm is crippled, you can't use two handed weapons. When 2 are crippled, run for the hills!
No Healing on Rest | FO3 Nexus | [FWE Incompatible]]
Removes the insta-heal provided by sleeping for 1 hour.
Equipment + Items
Ammunition
Ammo Weight | FO3 Nexus | [FWE Integrated] | [Mez's Pick]
Ammo actually has weight now! You gotta think a little bit more about how much you can carry. This mod is compatible with mods that adjust the rarity and/or value of ammo, as it works via a script. You might also grab DAV's Weighted Ammo Weight mod, which tags the name of ammo with it's weight so you can better estimate how much it weights. There is also the Ammo Weight Extended mod that adds ammo weight to CALIBR ammunication. Finally, there is the Displayed Ammo Weight mod, that displays ammo weight in your HUD, and which requires Darn's UI.
Ammo Toolkits | FO3 Nexus
Requires CRAFT + CALIBR. This mod lets you purchase schematics to make your own ammunition at the workbench. Fun little thing.
Ammo Schematics | FO3 Nexus
An alternative mod to the above, that lets you create ammo at new custom ammo workbenches. A CALIBR version is also available.
MrSlackPants ? Ammo Changes | FO3 Nexus | [FWE Incompatible]
Primarily changes the value of ammo. Good to use inconjunction with the ammo weight mod.
New Weapons

This is a great mod that adds dozens of new weapons that existed in the prior fallout games. There are a few rough spots in the mod, but overall the package is pretty good. New items appear in leveled lists on NPC's and at vendors.
Missing Unique Weapons | FO3 Nexus | [FWE Integrated]
There were many unique unique in the original game that weren't placed or used anywhere in the gameworld. This mods adds these weapons to special NPC's or locations for your looting enjoyment.
Armor
Fallout2 and Tactics Style Armor | FO3 Nexus | [FWE Incompatible]
An armor (and to some extent weapon) overhaul that re-creates some of the different armor properties that were present in prior fallout games, like specific resistances to different types of damage. Try it out eh?
MR Armor Overhaul | FO3 Nexus | [FWE Integrated]
MR Armor OVerhaul provides a fairly extensive overhaul of most of the games original armors. Armors are rebalanced so that most armor have a logical weight to DR balance and can serve a purpose depending on your character.
The Ultimate Power Armor Mod (TUPAM) | FO3 Nexus | [FWE Incompatible]
Major overhaul of power armor, including training, how power armor functions, its stats and effects, and more. Includes the addition of night vision to power armor. Very comprehensive set of changes. Will conflict with FWE unless FWE's PA change system is disabled.
Missing Unique Armors + Clothing | FO3 Nexus | [FWE Integrated]
There were many unique armors and clothing in the original game that weren't placed or used anywhere in the gameworld. This mods adds these armors to special NPC's or locations for your looting enjoyment.

This is a sweet mod, that adds tons of new drugs and chems to the game, new visual effects, and more. New chems are added to loot lists, so be mindful of potential conflicts. Very nice mod!
FDA Mod | FO3 Nexus | [FWE Integrated]
Another nice food and drug mod, that adds some new alcohols, new food items, and a few new drugs. Again, new stuff will appear in loot lists and elsewhere throughout the wastes.
Fallout Food | FO3 Nexus | [FWE Incompatible]

Well, this is my new favorite weather mod. Keeps the wasteland feeling gloomy and moody, but consistent with the original feeling of the game. Includes Xepha's Darker Nights (since this mod was also made by Xepha) for a good all-around weather package. Works with enhanced rain as well.

This is a great weather mod in my view. It keeps the same tone range and feel of the original game's weather (unlike Fellout which provides a completely different feel) but adds a lot more diversity and interest to what's going on.
Enhanced Weather - Rain + Snow | FO3 Nexus | [FWE Companion]
Adds rain and occasionally snow storms to the wasteland, which makes a HUGE difference in the environmental feeling. This mod is compatible with the Wasteland Weather Overhaul and Fellout to my knowledge, as it operates via scripts.
Fellout | FO3 Nexus
One of the more popular weather overhauls, Fellout removes the "greenish" tint to atmospheric style, as well as providing a much bigger range of weather types. I feel it makes the wasteland a little too bright and cheery, but that's because I like my wasteland dark and gloomy.

This mod makes street lights in the DC area actually shine light, which combines nicely with darker night mods to make things more gloomy and spooky. By the way, who turned the power back on?

This is a great mod, which has unfortunately grown a little big in my opinion. Regardless, the premise is that if the occupants of a house should experience an disastrous calamity, you gain ownership of the house and can loot it, move in, or otherwise exploit the property. The newer versions of the mod let you become a bit of a land baron, and charge people rent, kick them out, and other fun things.

Hides the skill requirements for disarming traps and performing other tasks. In addition, traps are less of a sure thing to disarm, and have a chance of blowing up on you even if you have sufficient skill to disarm them.

Makes it so that you can't see the [EMPTY] tag on containers with nothing in them. Realistic for sure.

Reily's Rangers Expanded | FO3 Nexus
Tired of the lackluster ranger compound? Well, in this quest mod, subsequent visits back to the ranger compound (after rescruing them) with more mapping data gradually causes the compound to expand. More rangers are recruited, merchants eventually show up, and more. Makes the journey out the compond more worthwhile and breathes more life into the Rangers.

Expands Arefu, adding new quests, NPC's, and more, making the town what it should have been in the first place, a certified little hovel.
