...or crippled head, or acohol abuse?

Or it simply hasn't happened to me yet after 60 hours of gameplay?

There is an effect, I've experienced it a few times from head crippling. Though I don't know if it is permanent or not. There isn't an option anymore to target specific appendages for the stimpacks, it just does general healing and when I get hit with the crippled head I quickly use a stimpack which I have slotted to D-Pad Down 1.

So whether or not it is there or not, I'm not sure because I've never let it affect me for more than a few seconds.

As for the drug addiction, I think it is there as well but I haven't been addicted yet. Though checking some Youtube videos, it does look like there is at least screen blurring for addiction.

Crippled heads definitely give blurred vision. I usually heal it immediately. There was one prolonged fight where I didn't heal it immediately and I believe the blur faded in and out approximately every 10 seconds.

Crippled legs give you permanent reduced speed until healed. So I hope the crippled head effect is permanent until healed. Even if it fades in and out.

On a side note. What is the effect for crippled arms? I assumed it was increased weapon sway, but I honestly feel like I'm a better shot when my arms are crippled.

Yeah your weapons sway dramatically. In first person the camera moves back and forth to simulate that.

You won't feel any effects from addiction itself, it is the withdraw that gives the visuals in the past. So check to see if there are withdraw symptoms (don't feed your addiction for a while). If anything they might have removed them, and just kept the stat reducers. Which really makes more sense if you think about it: the visuals usually come during the use of a drug, not the withdraw. Withdraw symptoms in real life are usually associate with uncontrollably trembling, shaking, sickness, lack of reaction time (worse than when on a drug), etc. All of those can be expressed in a stat reduction without the need of a visual effect.

I know, that was my point. 110 hours in and no withdrawals despite the initial stat reduction.
