what would happen realistically with someones head really depends on the kind of gun.... smaller handguns might only make a hole, larger caliber take a chunk, shotguns take a large chunk, anti-materiel (anti-tank sniper) could make a person's head explode like that, as well as grenade/missile launchers etc.

Theres already a mod doing this, its for FO3
but should be someone around who can do the same for FONV

From the description:

This mod is an attempt to make the dismemberment in Fallout 3 less ridiculous.
Having over-the-top deaths is great when they are relatively uncommon, but in Fallout 3 they happen too much and often make no sense.

The F3 engine distinguishes between limbs being severed and limbs exploding. Standard settings allow almost every weapon to do both,
with a 50% chance to sever and failing that a 75% chance to explode a limb on a kill.

There are two versions of the mod to choose from.

The normal version modifies weapons so that only certain weapons can cause limb severing or exploding.
For example, only large bladed weapons and explosives will sever limbs. Some guns can explode limbs, but only the large or powerful ones.
Also, the overall chance of dismemberment is reduced.

The lite version just makes dismemberment less likely. Individual weapons aren't modified, so there should be no conflicts with other mods.
However, it will still be possible for your low-caliber pistol to decapitate someone.

The result is that there will still be gory deaths, but they will be less common. Unless you grab the Bloody Mess perk, of course.

I think it was because they didn't have New Vegas Mod Manager, or didn't put it at the bottom of their load order.
