No, it's part of the vanilla main quest if you take a certain path in a conversation. Can't really say more in Gen. Discussion.

Accidentally killing myself with an Experimental MIRV. A hell of a fly, that was.

The Experimental MIRV is fun, when you use it properly.
Accidental self-kills using it, however, are a thorough going improper use of the weapon.
My own case of that happened in the Rivet City Marketplace. From some earlier events, I'd gotten hostile status in Rivet City. Gunfight with a guard in the Marketplace, and my 10mm SMG clicked on empty. I had the Missile Launcher set as quick select 3, and the MIRV set as 4. So whilst I ducked behind some cover, I went to press 3, and missed, hitting 4. As the weapon was being pulled from Inventory, I was waiting, planning on a snapshot for splash damage to eliminate the guard. As soon as the weapon was out of inventory I was out of cover and firing, NOT looking to see which weapon I'd selected...
OWWWWW!!
3 hostile traders, the guard, and myself. All insta-kill. :facepalm:

I don't count mishaps as fails but one moment I really regret was shooting two Enclave soldiers who had surrendered.

buying Fallout 3, I wish I had still had the 15 dollars, I could have gotten some cheap hokers with that.

not my most fail but one that comes to mind...

trying to do the THOSE! quest with a low-level melee character on hard difficulty armed only with the bat you get at beggining of the game.... the firebreathing ants one that battle. BUT not teh war! :stare: :toughninja:

Mine was when I walked into Old Olney and I didn't notice the dozens of Death claws that lived there... Lets just say that a dart gun can only work so well when your surrounded, and on top of that instead of armor I had sixy sleepwear...

I was getting chased by a large group of super mutants so I ran onto a bridge to lose them, turns out the bridge has a large collapsed segment that I need to cross and I had to go back to the super mutants

Mine was going to Rivet City for the quest, looking around the whole ship for a way to get in. I failed to see the intercom by the front. Until I saw a door that was Locked-Very Hard. I ventured off into the wasteland in search of XP to level up. 6 levels later I came back and found the intercom.

I did this exact same thing. :foodndrink:

I recognized the intercom...only due to the fact that I took forever to try to get into Galaxy News Radio at an earlier point and thought that it was a bug causing me not to be able to enter. :blush2:

Needless to say (after reloading the game just in case), I found the Intercom at GNR, and later recognized it for what it was at Rivet City. :)

My biggest fail moment in FO3 was when i first stepped out of the vault. I missed the (not obvious at all) big pile of metal called "Megaton", and decided that you had to get to the Washington Monument.
5 minutes into the game, i lost all of my ammo, died (on very easy) and ragequit the game.
Greatest FO experience ever.

Looking for Dogmeat for a good half an hour in the scrapyard on my first play through. Then, when I wasn't really paying attention (think i was googling where he was) he actually found me and gave me quite a surprise as he ran towards me.

Thinking 'It's coming straight for me', my obvious reflex was to shoot the hell out of him :facepalm: I didn't have dogmeat on my first play through...

I didn't realize there was an intercom to get into Rivet City either the first time I played.

I also couldn't find Megaton because I didn't know about the compass map pointer thingie. :facepalm:

I would say dying about 6 times in combat during Point Lookout before finally looking on the internet to find out that Feral Ghoul Reavers are tougher than 100 year old beef jerky wrapped around a coffin nail. I hate those things.

That's one of my memorable fails anyway. Did anyone else find the PL combat waaaay tougher than most of the capital wasteland? It was brutal.

Failing to open up the nailed-together pieces of cardboard that constitute the back gate in Little Lamplight, despite having a char with 100 Explosives and 100 Repair skills, and enough ordnance on me to start another world war ...

(I ended up simply bringing up the console and removing that door from the game)
