I usually play Ranked Team Instant Action because I assume this game type is the most popular. Almost 60% of the time, I am put into an empty game with nobody else in it. When I am put in a game, it's usually a French or Italian server and my ping is about 200 (I live on the west coast). Is it something with my settings that set me up with foreign servers, or are there just not many servers to join? I thought at first it is because my NAT type is set to Moderate, could this be the cause?

Forget this "Quick game" thing, it's useless. Go to the server browser and set the filters to your preference and sort the servers according to your main concerns (ping, game type, whatever).
