I don't have a problem with Games for Windows Live. It didn't ruin my game at all - in fact, I was more annoyed that I didn't bother with it on my first playthrough and missed out on all my achievement points.
I definitely think you should play it at least mostly vanilla the first time. If you don't know what the game should be like, you won't know what you think needs improving. Sometimes you'll find you prefer the game the way it was shipped - or notice a bug in a mod that you only recognise because you know how it's supposed to look. (Reminds me: I need to work out why a wall is missing in one cell - must be something from a mod. It's definitely not supposed to do that!)
House mods are normally pretty OK, and very small tweaks - but just avoid major overhauls until you've played the game through thoroughly.
Also, if you're going to play with mods, don't get too attached to savegames. The game is inherently designed to be played over and over anyway, from different angles - so always be prepared to start a new game when you need to - especially with the wider use of .esm files in mods these days (they're pretty hard to uninstall).
More normally with the game, the first time you play it it's best to just take the hint: go to Megaton first, talk to Moriarty then Moira, and accept the local quests so that you are gently introduced into the world. It's a game with a definite ending (without the Broken Steel DLC), so you'll need to do all your sidequests and exploration before you get too far into the main quest. It should be fairly obvious when you're getting near the end - there are a few clues that you should finish off what you're doing if you want to before proceeding to the finale.
