Shouldn't the realm of madness be a more twisted version of the world (but still a version of the world) anyway? And to have mortals caught up in it seems fitting too. Maybe they're even in two places at once, and you just happen to get caught up in the realm by being mad.. or get mad by being caught in the realm.

The people in SI is there with their own free will thou, they haven't been caught or anything.
