Nose job : Fallout: New Vegas

My auto doc said he could give me a nose job, but the only options he is giving me is heal everything, hair cut, facial restructer, implants, and I think that's it. How do I make it give my girl a nose job?

You even listed it. Facial restructure. You change your whole face (including nose) however you want.
