I am having a problem with the Ice Form shout. I received the first word from the wall in Saarthal, and I went up the the wall at Mount Anthor while fighting the dragon and I got the typical learning effect, except it didn't add a second word to the shout. I went back to the wall in Saarthal and all the audio effects: chanting, music and noise are there as if a word was on the wall, but no word is glowing word and nothing happens when I go up to the wall. I figured I'd get the third word and it might fix the problem. In Frostmere Crypt, it is the same as Saarthal with the audio effects present, but no glowing word and no reaction to approaching the wall. It svcks cause this was on of the shouts I actually wanted. I haven't been to any other walls that give second words yet, so although I am disappointed that my Ice Form is bugged, I really hope the same thing doesn't happen for other shouts.

Edit:
I have gone back to two different save points. When I go back and get the word from Anthor again, after already having the word from Saarthal, the problem again occurs. If I go back to before I got the word from Saarthal and get it from Anthor first. I can get both words.
It seems to me that the problem my be caused by the fact that each wall has a word, not 1/3 of a shout. I collected the words in the wrong order and thus a glitch occurs and I am locked out of the rest of the shout.

Im having the same problem as you except that i originally got the first word from Mount Anthor and the proceeded to Saarthal where its gave me the third word "statue" instead of the second word "flesh". the when I checked the shout nothing was available to unlock so I thought I just had to go the the location with another word in the shout and it would give me the second word. I cleared out Frostmere Crypt and when i approached the wall it played the music but the word was not glowing and it didn't teach me anything else. Im not sure how to correct the glitch and if there gonna patch it but I had 50+ hours on the charter and now there a glich with a core aspect of gameplay and i dont wanna continue playing if it might happen with other shouts. Hope the patch this soon.

I guess until its patched it would be wise to save before getting any word, and if you see it is a third word, reload and avoid.

There are 2 different ice related shouts that I've found so far. I thought the word would add more power to the ice shout but if it's the word I'm thinking of then it's a new shout, did you check and see if the shout is in that section and needs a soul to activate it after approaching the wall?

That's what I've been doing since I found the problem. I save before going up to the wall and then see what I get. I am hoping that is gets fixed cause I was really looking forward to freezing a group of people for 60s with the full Ice Form shout.
