I have had this bug a lot recently. Whenever I go in to V.A.T.S and there are a lot of enemies on screen, like at violet's fiend hideout (this happens every time I go there), my character will not shoot in V.A.T.S. This is very annoying as I means I'm using Stimpaks that I would not use otherwise (I have not died yet). If anyone else has had the same bug please tell me
