As stated Farkad is running around with out at a Chest piece. Has wolf chest piece in inventory.

Merged patch? Bashed patch? Preferably both, in that order?

I've had the same issue with various NPCs and thought it was because of Everybody's Different, but I see now it isn't. While I don't know what's causing your problem, One, you could try reseting Farkas by opening up the console, clicking on him, and typing resurrect 1. That will "kill" and respawn him with his original equipment, which he should then equip. If not, hopefully someone else will have a better idea.
