Wait, do you mean the official names of Dagon's and Sheo's realms? If so: Madhouse (Shivering Isles) and the Deadlands.

Or do you want us to name every realm, or something? Sorry if I am mistaken.

It is simply called Umaril's Sword and the base appearance is Elven.

The enchantments are shock damage and damage fatigue, I think.
