They totally blindside you with it. I figured out who he was before the reveal based on his hideous features that he inherited from the joke spouse I made, but you only meet him like 1 minute before the truth is revealed. There really weren't any clues, in fact the game seems to contradict itself. For one, Kellog hasn't aged at all since Shaun was 10 but we know he's not a synth and if the institute has some kind of age defying tech, why does no one else there use it? Shaun obviously didn't have that kind of tech and Dr. Li has visibly aged since Fallout 3 as well. Then you have the people of Diamond city. They all remember Kellog and the mayor wants to respect Kellog's privacy and not let you into his house. If that was 50 years ago, most of the people you talk to wouldn't have even been alive yet let alone remember one random guy and there's no way they'd let a house stay vacant for half a century. And of course you have the whole chase with Dogmeat. There's no way a scent trail would survive for 50 years, especially out in the wind and rain. None of it adds up. Either it's really sloppy writing, some experiment aged Shaun prematurely, or that's not Shaun.

Kellog explicitly states in logs that he is being kept alive and "young looking" by turning into a full on cyborg, which is why he had so many parts on him.
The Mayor is a Synth and works for the institure, so he would OBVIOUSLY not wany anyone in the house, even after so many years, as a just in case,
most of the people who remember him are generally not exactly young or if they do know him, it might be by rumour, or simply that he DOES go back to the house every once in a while, even then, it is entirely possible that the kid we saw him with was the Synth version of Shaun.
I wont say too much, but that wasn't Shaun in Kellogg's memories.
Replay that section and listen to the radio playing, its Travis talking about Piper's article about the mayor of Diamond City, the one that got her kicked out not too long before we get there in-game.
The cyborg thing just seems weak to me since nobody else is using that technology and it seems really useful. He seemed fully human. Kellog having one of the Synth reproductions is more compelling though and would explain why there's a fresh blood trail and why everyone remembers "the guy and the kid" living in that house.
They do explain in a terminal entry in The Institute why they didn't use that tech on others.

I just remember reading about it quickly while I was looking around The Institute before I murdered everyone there for being scum.
