Right now im at -2 perception and it continues to fall. What am i doing wrong?

Not at all, it keeps falling. It was at 4, then 2 then 0 now -2 and it keeps dropping. Im afraid I have to make a new character. Everything goes double vision every 15 seconds so i cant get anything done.

Anyone else having this problem?

Well i saw a doctor from a traveling caravan and it says i was healthy

maybe bad radiation is affecting it?

mayeb you can take a screenshot of some relevant screens? (pipboy stat screen showing general health, the -2 perception stuff etc. etc.)

Is your head being crippled still a thing in this game? What you are describing is a crippled head, which causes blurry periods frequently and reduced perception.
