It is the Cerberus Helmet with Beret from FOOK2.

Well, lets see...I've missed...9 threads; far too many. I will soon be returning to the realm of Fallout 3 after some hardware issues and then a computer-crash-induced hiatus; so alas, I have no new screenshots. But my shiny new machine will be arriving shortly, and new screenshots will then be posted!
