I can recommand Arwen Realism Tweaks and Fallout 3 Wanderer's Edition. Both make hardcoe style changes. Probably much more hardcoe than FNV (I didn't really played yet so I don't know).

Maybe FOOK2 also?

I'm using Arwen's Realism Tweaks and it definitely changes the game and makes it a lot harder. You require food, water and sleep. Radiation sickness is a lot more serious. Injuries are a lot more serious and if you don't take care of them, it can lead to infections. The mod changes most aspects of the game. Check out her website and she goes through, in great detail, other compatible mods that she uses.
