Try deactivating All Natural. Your GPU might not be strong enough for it.

I'm not sure I agree with that assessment. Real lights module maybe. The bigger killer is probably the 'actors have torches' esps. They wrecked my fps when I tried them. Try just disabling those and/or adding the 'no light flicker' tweak to your bashed patch. Have you tried Stutter Remover? Might help.

Also, have a defrag, never hurts. ;)

Almost same specs has you Amd x2 4000, 2 gig ram, ati 3650 512..Xp Sp3..

Remove IWR and use AWLS - Animated Window Lightning System Instead

Remove OOO Waterfish that is killer on frame rates has well..

Actors have Torches are killer on frame rates has well

Short Grass replace with RPG BlackDragon Tripoly Grasses mod instead..

RAEVWD is killer on frame rates and can cause stutter has VWD objects are loadedd etc, but it most optimized, I use it, but, I do not use the plugin it will help alot. Installing with either OMOD or BAIN and Choosing on Fort, Ruins, Cities, Homes, Ships, etc..

My Advice

No Lights Flicker under Tweak assorted when you rebuild your patch

For RAEVWD, yes all the _far.nif files are probably still in your Data folder. You could try the following Oblivion.ini tweaks -
uGridDistantTreeRange - Default value 15. Setting this lower will reduce the visible distance on trees, and potentially improve frame rates, at a cost to realism. Raising it higher obviously increases the view distance on trees, potentially reducing frame rates for increased realism.

uGridDistantCount - Default value 25. Controls viewable distance for LOD buildings, ruins, etc. Especially useful when installing RAEVWD as lowering this to 20 or 15 will dramatically improve frame rates without having to turn distant objects off completely.

...credit to Arthmoor and Tomlong. I have these set to 10 and 10.

install http://www.tesnexus.com/downloads/file.php?id=10400 and adjust the fog values in sl.ini to match the above settings

Good Luck

would the dungeon actors still have a hit on frame rate even though they are in a interior type cell?

Yes, all cases of realistic lighting (torches etc) do so. Although interiors can often be less of a hit in general (smaller space, less stuff for your machine to 'read',) so it may be that the hit is less important in dungeons etc.. Check out your frame rate outside compared to inside caves etc.
