It happened right after I leveled up. It's one of those random faces that you see when you have to "escort a character." I tried reverting, it happens only after I reach level 10, and the face looks different every time I revert.

Stupid question, but do you still have an escort quest active? Or did you fail an escort quest while you still had the NPC with you?

Otherwise, maybe the face will disappear when you get to level 11. I never heard of this funny glitch.
