How does being fat work?

Provided Bethesda has kept the .nif format, the way body-types will work is not by modification of the mesh, but by modification of the bones. See this mod for Oblivion:

You can change the body shape of your character at will with that mod. How it works? Basically, meshes stretch to fit the shape they need to be, as designated by the animation 'bones'. In Skyrim, Bethesda will probably put limits on the sliders, so you can't make your character so fat you distort the meshes/textures too much.

But, but, I want jiggle bones. :D Haha, not necessary.

it would be cool if you used fast travel alot and didnt do much + ate a bunch of food your player would look fatter, then you can go quest off those pesky pounds

Why would using fast travel make you fat? You're character still walks when using fast travel, you just don't see it. At least in Oblivion's system, if it was like Morrowind's travel services, than that might make a bit of sense.

But in any case, I'd assume body types are just an option you can change during character creation, and I hope so too. I don't want the appearance of the character I want through great lengths to get looking the way I want to change suddenly due to things outside of my own choosing. If there's a way to change the character's appearance after character creation, it should only be a choice available to the player, not a natural progression caused by things that happen during gameplay which players cannot directly control.

This!

Setting up a character to be fat is one thing, an option that players should be able to choose on char creation. Once that's done, it should be set - you wont find Weight Watchers on the menu at any tavern, no one talks of Diets in that day and age.

I personally doubt we will have the ability to make characters very fat as that would pose a big challenge for game-armors (which have to fit snugly around the body in order for it to look-right with all the motions the player must make). They already have to size the armors to fit the different races and potential heights, I can imagine it would be quite a bit more work to add variable width the each armor selection as well.

I think it would be nice for players to have the option, but the armors may limit this flexibility.

Same, and seconded!
FT (should be changed to "Foot Travel" for clarity sake), if anything it should make you lose weight.

The game 'Redneck Rampage' had the PC get fat if they abused food and drink; it would also make them flatulent ~which inhibited sneaking around, by attracting attention, (and occasionally could knock you off a ledge).

I'm more interested in how tall or short characters can be (without being a specific race)

A height slider is in order.

As for weight: instead of a sprint option you should have a roll option.

Realistically though, back then it was hard to get fat even if you did laze about a lot. Your job was exercise in and of itself, as was getting there, and all you got to eat was two meals comprised of leeks a day.

The closest you'll get is a beer belly if we're going for realism, but I say do it anyway. Variety is the spice of life after all. No, I don't mean a curry you greedy git!

And I swear if we're going for a slider that changes in-game by itself depending on dietary habits. It had better not follow the "I'm fat because I ate something more fatty than a no-meat sandwich" rule of "vegetables make you thin; meat and everything else makes you fat". I'm virtually carnivorous and any fat I have is from too much junk food and not enough exercise, and that's given I only look even vaguely fat when I sit down. I look a little like an accordion.

This wouldn't PREVENT jiggle bones, it'd just allow you to reposition the mesh around the bones, essentially, to make the mesh bigger or smaller. (In your case, I'm guessing you'd want bigger. )
