Well... more black hands? I have to say no, even though it's not really an important question to me.
The reason I say no is because these higher ranks should have fewer people, thus you respect them more and think of them more as unique.
That's my way of looking at it at least.

No to the first question simply because the Black Hand is supposed to only have five members, just as a normal hand only has five digits.

Yes to the second question, cause there should be more than one sanctuary for the DB, and the top members need a place to hang out.

Like some here have said, Black Hand only has 5 members. So it wouldn't make sense if there were more.

However, I'd be more than happy to have more Dark Brotherhood sanctuaries and members, and especially more quests, as DB was and still is my favorite faction.
