Interior lag

I am running an above average rig with an ATI Radeon HD 5700. The game runs very smoothly outside, but once inside the game starts to lag badly. Ironically the smaller the room the worse the lag gets. This can be slightly offset by switching to third person, which for some reason reduces the slow down. I had this problem before and after the patch and I am running no mods. Also changing any of the graphic settings to low, ultra or any combination between has no discernable affect. Also the dll. file had no affect on this issue before and after the patch. I was wondering if any one else has run into this problem and do they have any ideas how to fix it.

i got this problem after the patch along with micro stutter but with the Dll file before patch never had these problems

I sometimes have lag indoors though its fixed when I turn off any radios that happen to not be emitting sound though are on. (happens randomly as they work sometimes and others not) Also save and reload in some places might work.

I'm getting sudden bad microstutter indoors since the last patch. Alt-tabbing out and back in again clears it for a while.
