Do you have a single core processor? If you do, the only trick that seems to work well for me is to ctrl-alt-del to the task manager and set the process of falloutnv to high priority. Also I'm on xp. Also you may have to alt-tab out of fonv in order to have that process in the processes list.
let me put it this way, this is what I do to get my sounds working right:
1. ctrl-alt-del to get task manager up and running
2. run fonv
3. load save game
4. alt-tab to task manager
5. click process tab
6. find falloutnv.exe
7. right click on it
8.click on set priority
9.click on high or above normal
10. you'll get a popup asking if you really want to do this, click yes
11. alt-tab back to falloutnv
12. I generally get some weird sounds after this, so I reload the save
This works for me on xp, I choose to load a save first because it seems to destabilize the game more doing it from the load screen. Also, this fix isn't perfect, Setting the priority higher (at least on my single core machine) will eventually (or so it seems) lock up the processor after a few hours.
