The actual mechanics of stealth itself doesn't really need an overhaul, as much as enemy AI when reacting to stealth does.
In generally, it just needs to be a lot harder to sneak than it is in say, Fallout3. New Vegas was a little better, but inconsistent. Generally, if you kill some intelligent entity (Men, Mer, whatever) and the actual kill is witnessed, there should be no "Oh, frank is dead, let's go back to normal" situation. They should search relentlessly. Reacting to dead bodies would be nice, but probably unreasonable to ask for.
Unintelligent entities, like most animals however, are generally fine. They see an animal die, and they might get startled, but otherwise, can resume standard behavior patterns.
As for what changes need to happen to criminal activity, that's a whole different story.
