If you can have the option of becoming a ghoul or a super mutant, it will change the entire story and gameplay balance. First of all, most of the NPCs will have to interact with you differently, some areas might even not allow you to enter all together. Plus there will have to be an entirely new set of perks and skills for your player. Finally, since as a ghoul you will now actually be healed by radiation, basically giving you the Rad Child perk from the very start, there will also need to have some serious negative effects of being a ghoul in order to make the game balance.

But back to your question. I think I will be a pre-war ghoul.
