in fallout new vegas since you don't start in a vault or as a child you could choose to play as a ghoul in fallout new vegas as a ghoul you would be healed by rads and feral ghouls would'nt attack you and you'd gain some sort of ghoul speech option but it would be harder for certain factions to like you, your limbs would be injured easily since your already falling apart an maybe some random run-in with bigots that attack you.
