I've changed a few settings such as mouse sensitivity default keyboard key and they don't seam to save even after hitting apply.

now some keys have also stopped working in game, e.g I changed melee to mouse button 4, in game to advised to use V for melee but neither key works. Tried to change the key and still does not work.

Same exact problem, I changes settings and nothing changes at all.
