This may seem true but it's just due to the fact you're paying attention so closely that you remember missing.

I was extremely surprised when I noticed one day that my headshot of kills is little over 50%, I definitely knew that I hadn't had so many headsots because u see a little head icon when u get one. Then again I often wondered why the hell I die so fast without a headshot icon showing that they shot me in the head, so I figure that the icon shows only when you use one bullet to kill a player but when u shoot them anywhere else first and then in the head it doesn't show the icon, so what's your headshot to kill ratios?

I'm not sure, it was very high when I was strictly using the sniper. That was recently after the release, though. I've used almost every gun since then. I personally find that pistols are the best for head shots.
