Was someone using the xbox before you at that time? If a profile gets loaded, and that profile plays for a while, and then you switch to your profile, with the game start menu still playing in the background.... you must restart the game.

Do you move your profile around a lot? On one of those memory sticks I mean. Did you get the DLC on a friend's xbox, then move your profile back home? This means that the DLC you purchased is only licensed for your friend's xbox: The DLC looks at the box origin, not the profile.

Is your dashboard updated?

Off the top of my head....

Did you/someone perform maintanence on your Xbox 360 hard drive? (This means it deletes DLC saves etc and brings them back, Reduces Lag, Freezing and Overheating). If so then start a new profile, Mess around on it for about an hour or so, Then go back to your old one. Not %100 on this one but it's happened to me on F3 and Oblivion after maintanence.
