Series of fortunate events

Was doing a little bit more of the main quest at the weekend and I escaped Rave Rock and took a bunch of Enclave PA and Tesla Armour, I met Fawkes outside but he wasn't recruitable because of RL-3, who had pulled a Harry Houdini, so I continued to Megaton and here I decided to repair all my gear I had stored away.
After a few mins repairing everything I decided lets wear the Chinese Commando hat then I put on the Confederates Hat, I noticed that the CC hat hadn't been unequipped so then I equipped, Power Helmet, Shady Hat, Eulogy Jones' Hat and countles other that had effects and now my PER is at 10, CHR is at 10.
I tried this for the armours aswell and this was also happening, I had Chinese Stealth suit, PA, TA, Hellfire armour Vault Survivor and countless other and go my LCK to 9 END is at 10 but only the Stealth suit is visibly woren but my hat is changing wiht each load and some are meshing with each other or my Stealth suit, like the Raider Psycho-Tic helmet and the Stealth suit and this looks like the futuristic http://fallout.wikia.com/wiki/AntAgonizer%27s_costume
Another thing...Am I meant to have 2 companions at one time ? Because right now I have, Fawkes and Paladin Cross, I got them in that order after Fawkes went to Underworld or is it like NV were I can have 1 non human and 1 human ?

Has anything like this happened to you, without the use of mods ?

You can have one companion plus Dogmeat - not sure how you've got two companions without using mods!

I could get two followers and dog meat as well sometimes on 360.

The hat glitch is a known one, wearing the CSA will allow you to put on an infinite number of hats (but not more than one helmet, I think). This is actually part of godlike unarmed glitch (you can bump your unarmed skill up so far that you can one hit anything in the game without having to wear or carry a ridiculous number of items). I don't think I've ever heard of a similar glitch for armor.

There is actually a way to get all of the followers at once, and it sounds like you've stumbled upon part of that glitch. I've never done it myself though, as I prefer to wander the wastes alone.
