I'm curious as to how you guys approach combat in this game.

Never for the past few characters as they've been melee fighters. Prior to that, unless using a scoped weapon, shotgun, explosives or the Metal Blaster, I always used VATS. Sometimes it's out of laziness, sometimes it's out of self-preservation, sometimes because that's my RP, sometimes it's just because it's fun.

Glad you included option four, since that's precisely why I use a lot of VATS

IMO the poll needs another option. (Don't they always :laugh:)
