Tracking your Death

After spending hours trying to find an answer, with no luck, I decided to come here an bother you guys again, in the hopes that someone might have an answer.

I'm working on a "death" penalty option for my mod. So far I have it working as a script that kicks in when your health gets really low. And that part works well.

The problem is that anything that damages a large chunk of HPs will instantly kill you, and then the game will reload your last save. I've countered that to a small extent with progressive saves, but it is still not what I was hoping for.

So here are my questions:
1.) Is there any way to track how many times the PC dies? If so, could that information be used in a script as a condition?
2.) If #1 isn't possible, is there a way to extract the name of the loaded save, after loading that saved game (after your death has automatically caused your last save to reload)?
