"How big" depends on your hardware. QTP3 ist known to be one of the BIG fps drainers. That said, with a somewhat decent-and-up-to-date-rig it shouldn′t be a problem - IF it is used more or less on it′s own, i.e. not together with several other major fps- "killers". Actually, bogging down a fairly decent PC with a single mod is hardly possible (still possible with VWD-options and the like), but if you combine several of them that′s a complete different story. If you want to use QTP3 AND Better Cities AND one or several overhauls (maybe even FCOM) AND NPC-adding mods like Crowded Roads AND VWD... - then you′ll very quickly have problems. Or if you′d just use QTP3 and 4096-LODs....
With my GTX460 (1GB), 4GB RAM, and Phenom X4 955 (a standard up-to-date rig about a year ago), I didn′t feel QTP3 at all (and of course it looked gorgeous), however I′ve tried it almost on it′s own. On a fairly modded-up game, it DID make a difference, well playable, but I didn′t have my framerate constantly at 60fps (capped) anymore, in fact in some areas (the dreaded ones, like IC harbour) it sometimes was well below 20. Still definitely well playable, but it makes you think about what to install next...or how it will look like 200+ gaming hours later when things generally start to get slower.
You could try "Qarls Texture Pack Redimized" which apparently has almost no loss in quality (opinions about that somewhat differ, and I myself didn′t do too much comparison-testing regarding this - I couldn′t see a difference at first (and second) not-too-close glance) but less fps-drain. Even further reduced is QTP Reduced (several flavours), all available on TesNexus. Some say combining QTP3 and QTP3 Redimized is even better, because one seems to handle more things than the other.
But QTP3 doesn′t only severely improve standard graphics, but also changes it′s style somewhat, and not to everyone′s taste. A popular alternative (and my personal recommendation) would be a combination of Vibrant′s textures and Bomret′s Detailed Normal Maps. That combination isn′t only much friendlier to your resources, but keeps the general graphics style closer to vanilla. You can still maybe add a few other, more specific texture replacers on top of that. And there′s the OBGE Mod (Oblivion graphics extender, see thread here around) with it′s addons (like Godrays, Liquid Water and various other great shaders), which might be a better choice to use your remaining system′s resources on. Some claim that′s even better looking than QTP3...
Finally, I would add OSR (Oblivion Stutter Remover) together with the LAA-4GB-patch (if you have at least 4 GB, albeit this patch makes more sense if you have > 4GB).
If I was you, I′d add the texture replacer rather late on a new installation, and would backup the folders used by those mods before (best option would be backing up "meshes" and "textures"). Then I would just test it out if you have sufficient resources for this or that graphics solution.
