So the only settlement we've seen in moderate detail is Riverwood. OB started in the imperial city, which was fairly appropriate but where do you think Skyrim will start? We do know that The PC is being led to his/her execution but thats about it.
This is just my crazy theory, but since the game always gives you a way to start the main quest right after you make your character, I think that on the way to being executed, something will happen with a dragon. Maybe it will talk to you, and this will convince everyone that you are dragonborn?
It can't be in any major city as we know the location of the exit of the beginning cave thanks to the placement of characters during the demo and it doesn't seem to be anywhere near a major settlement. Other than that we know nothing.
I hope it starts in a fairly remote area maybe in a small town where people would naturally be wary of outsiders, which would provide a simple explanation as to why the PC is going to be executed (something happened and the new guy in town got blamed). Also when I find my first big city I would like to have another feeling like when I found the city of Vivec in MW.
Prison, It's will most likely be in a prison on your exacution day.
Oooh that's a great idea, I never thought about having a prison in the wilderness/away from society where they keep the worst of the worst--usually the prisons are only in major settlements in TES games, but this is a good theory.

They never give a reason for why you are in prison, obviously for the role players.
