Are dungeons randomly repopulated? : V

I remember doing a dungeon with level 10. Now I'm 37 and an NPC send me to the same dungeon to get his stuff.

The dungeon was repopulated, so was the loot. Even the chests were new.

Does this only happen if a NPC sends you there or is it independed and happens all the time randomly?

i havent re-visited a dungeon i have been before yet without a quest there
