Please help — loading area slows PC over time, then crash

I am getting consistent crashes when traveling medium-long distances. At first the game is "loading areas" fairly well, then this process clearly becomes more work (though FPS is unaffected post-load), and after a time it crashes. Has the cumulative feel of a memory leak.

I am running a bunch of mods (I'd list, but I'm not sure how to do the list so it is able to be collapsed — an explanation would be appreciated). I'm also running Streamline, which doesn't seem to help much. I've followed some of the settings tips, and none have fixed the problem.

My PC isn't low-end, but it isn't new tech either:

CPU: Intel Core 2 Duo E6600 @ 2400 MHz
RAM: Corsair 2GB Kit DDR2-800 XMS2-6400 Xtreme Performance Memory
GPU: Gigabyte GeForce GTX460-1 GB GDDR5 PCIE2 GV-N460OC-1GI
HD: Western Digital Caviar SE16 WD2500KS 250GB Serial ATA II 7200RPM Hard Drive w/16MB Buffer
HD2: Western Digital 1 TB Caviar Blue SATA 6 Gb/s 7200 RPM 32 MB Cache Bulk/OEM Desktop Hard Drive - WD10EALX
Sound:CREATIVE SB AUDIGY 4 SE
OS: Microsoft Windows XP Home Edition 5.01.2600 Service Pack 3 x32-bit
DirectX: 9.0c

Any tips on how to avoid this or, if this is a RAM clearance issue, how I can effectively exit the game, clear the cache, and then go back in every so often would be greatly appreciated.

First off, are you using a program such as OBMM or Wrye Bash to manage your mods?

Use the button with two brackets ("insert code snippet") to create a collapsible list
