I think it's a bug related to spawning when an area loads. I went out on an outing, and when I returned I found the dog in Riverwood dead (think his name is Stump, or something along those lines). I reloaded to an earlier save before the outing (when Stump was still alive), repeated my trip, and when I returned the dog was fine.

The named dragon bones (the first dragon you kill by the tower in Whiterun) disappeared on me after a week of remaining near the tower. I was running around the area wondering where it went and suddenly the entire dragon skeleton (still named) fell out of the sky and landed near where I had originally killed it. I think sometimes that happens to MOBs that load when you enter a region.. sometimes they bug out and load in a state or elevation that ends up being fatal. In fact, I recall reading a review around launch day that complained about MOBs spawning in the sky and falling to their deaths. I would not be surprised if something similar had happened to the dog Stump in Riverwood.

NEWS FLASH:

Lydia the sworn protector of the Thayne of White Run has been killed. Apparently she decided to steal a sweet roll and the guards some how figured it out and killed her. It is unclear how they received this information, which was merely minutes after the supposed theft. Also adding the confusion is the almost supernatural ability of the guards to act on this information and not know Lydia lived at the residence, making the theft no more illegal than having breakfast. The body remains unclaimed and no investigation is being conducted. No word on the Thayne's condition.

You can move bodies around, so it really woulnd't be hard to "fake" the scene if he wanted to.. I doubt the OP is lying, what would be the point.

I'v had a companion completely disappear out of thine air once. I found a mage in the forest and had her follow me to the Collage. I told her to stay there and a few days later I came back and she was no where to be found. So dumb.

There are definite spawn bugs going on. I've seen it with the dragon mirnilmur or whatever his name his at Whiterun. Bones dropping from the sky or just appearing, jiggling on the ground. Even had him spawn hunched over one the towers...jiggling.

Then, i'm reading of all these dead Lydia spawn bugs.

It's, frankly, creepy....these spawn bugs.
