Weapon moves off top of screen when crouched and not moving

Bearing in mind this is all with the camera in first person mode, WHILE CROUCHING. What is happening: After loading up the game initially, everything looks fine. Then at some point usually within minutes of moving, jumping, looting, shooting, inventory, i.e. playing:
  • If camera is held still for a fraction of a second, the gun will disappear (this happens with rifles, pistols, fists, etc.)
  • If mouse is moved, gun returns to normal location from the top of the screen as if the character had been standing up
  • Once I stop moving the mouse momentarily, the gun will disappear again
  • When gun is off the top of the screen and is fired, the bullet will hit a good deal above the crosshairs, as if fired from standing height
  • If the gun is transitioning between these two positions and I move during this transition, it will stay at that point in the transition i.e. halfway to the top

Some additional observations:
  • If I go from standing to crouching, my gun will remain stationary and fire correctly UNTIL I move my aim and hold it still again
  • If the gun is off the top of the screen while crouching and I stand up without moving my aim, the gun will remain hidden
  • If the gun is hidden after rising from said crouch, it will be brought down to the correct level if I move my aim. Alternatively, if I fire, it will immediately appear in the correct position.

Sniping is just not convenient when this is happening because I cannot hold the cursor still for a headshot without my aim being offset, not to mention having my arm and weapon constantly streaking across my view. Quitting to the main menu and reloading will correct the issue temporarily.

Yes, I have mods installed, but I am putting feelers out there because I couldn't find reports of this on this forum.

I have found other reports of this behavior:

2. This one is irritating. My guy will be holding his gun normally, then he'll just randomly lift it up higher or lower it to a point where I can't even see it on the screen. Also, when reloading while zoomed in with a scope, zooming out will cause a delay with the reload animation. Irritating.

xbox360 - When crouched and holding a rifle while in first person mode, your arms will occasionally appear to be above your head as if the camera moved down your chest and arms did not follow. It can be solved by standing and crouching one or more times. Additionally, the gun seems to disappear altogether from view, even when holstered and redrawn. It can be solved sometimes by switching view to third person and back.
xbox360 - Sometimes the players hand and equipped weapon will simply vanish from the screen as if it had been holstered but the player is still able to fire. The pip-boy also becomes invisible at the same time. Movement causes the players hand/weapon to reappear and disappear erratically. Quitting to the main menu and reloading usually solves this.

This happens across all the platforms (PC, PS3, 360). I've seen this issue brought up many times, but I have yet to see a definitive answer. This hiccup drives me to tears when I'm "really" into the game. It snaps me right out of any semblance of immersion.

Yes, it is driving me crazy as well. Sometimes it happens within a minute of loading up my game. It's really cutting into my enjoyment of an otherwise fun game.
