DX11 Issues- Black Border & Mouse Pointer

I downloaded and installed the 1.9 patch, DX11 patch, and high resolution patch. When I turned everything on, there was a 1" black border around the game and the mouse pointer was highlighting and clicking an inch or two above and to the left of where the mouse pointer was actually shown. Any assistance would be appreciated!

My computer specs:
AMD Phenom II X4 955 3.2ghz processor
Gigabyte AMD MA790XT Motherboard
ATI HD5870 video card
4gb PC1333 Ram
Razer Lachesis mouse
Razer Lycosa keyboard
Windows 7- 64 bit

I just wanted to add that I too am having the issue with the mouse and I have not found information anywhere on how to fix this. It also happens when I turn dx11 on in WoW and there is no answer (or response) on their forums either on how to fix this. For now I have both games reverted back to dx9, but I'd like to take advantage of my gtx580 with this new patch...

Also worth noting, I did not have any underscan as above post, but I did go into windowed mode and 720p (down from my usual 1920x1080). I fixed that by (painstakingly) navigating to the options screen and enabling full screen and 1080p. Also worth nothing, in 720p the mouse cursor is less "off" and closer to the actual selection than when on 1080p...

Also worth noting: NVIDIA control panel settings are set to default which mostly allows for the 3d app (crysis 2 in this case) to control settings.

Computer specs are not an issue...
