Rex the dumb cyber dog.

I was out near Novac, when legion assassins came out of nowhere. Rex and Cass were with me, and i desided to use my Holy Hand Grenade. well Rex had the genius idea of attacking before i initiated the assault (set him to passive, didn't work O.o). so after big explosion, Rex is dead and his only remains is his brain. I later found his body inside the mouth of the T-Rex at Novac dangling, limp, and worthless.

I got Cass killed by forgetting to remove a gas bomb in Novac and had the town riot. i watched Cass as she helplessly tried to fight off 4 farmers, 3 mercs, 2 merchants, and no bark. she killed nothing XD.

so i ran like a scared girl right into dead wind cave, and was undoubtedly [censored] to no extent.

That is why I don't tend to run with companions... They just die to quickly when they don't adapt to my play style.

Chucking grenades at your enemys when your dog is biting their ankles tends to be a bad idea.
