Poss Patch 1.3 bug : V

Just got achievement award on Steam for finding all 13 Standing Stones.This happened as I returned to the very first one at the start of the game.It seemed to treat it as a new one,even though it showed as found on my map.Thing is,I've only found 8 of them.Is this a possible bug?Anyone else had it?
Have also noticed since the patch 1.3,when I went to do smithing or alchemy or something similar,the instruction screen popped up,like it does when you first start the game.All my stats seem correct.Game is fine otherwise 112 hours in.Any similar moments for anyone?

You'll find that the patch reset a lot of flags... like the tutorial menus.

Had that happen ages ago, and I'm still on the 1.1 patch. Game lists me as having found 16 standing stones.

Not a gamebreaker or anything,still love the game for all it's flaws.
