Whatever happened to Vault 13?

Yes, if you had bloody mess, you shot the overseer as well as if you had bad karma.
No, the vault dweller, with residents of V13 and possibly his close friends like companions such as Ian or Tycho settled Arroyo.

We have the vault wiki for a reason, can't we just use it and not speculate?

OP, I'd say it must have been found by NCR and made into a settlement if not dismantled into a Vault City-like area.
Oh, you were right, I just had gotten my point of view as I started to replay FO2 a few weeks back and I misunderstood the intro.

Technically it didn't require Bloody Mess or Bad Karma, (though it was intended to). When I first discovered that you could shoot the Overseer ( years afterward ~like 2001 or 02), I did so because I happened to press the attack hotkey while entering the vault cave on that last trip home ~and there he was standing at the door... I really thought it was some sort of easter-egg at the time.

I still have never played a serious session with Bloody Mess as a trait.

(If you encounter him this way it's technically a bug, and you have only your current AP to kill him with (and he dies as usual), or the game ends ~and it does not show his death animation); there will be no 'next turn' in the fight.

Didn't know that. It's been awhile since my last playthrough, so I didn't remember if you were able to attack regardless of bloody mess/karma.

It'd be part of NCR, left empty, since they'd have killed the Deathclaws, and occassionally picked over by hopeful scavengers looking for rare and valuable tech.
