Walking mostly, and no fast travel helps extend the gameplay...as well as a few mods like New Vegas Bounties.

I found that walking everywhere added a whole new dimension to the gameplay, not as much as hardcoe mode, but still. It was tedious at times, but I enjoyed getting to know the Mojave like the back of my hand. By the end of that playthrough, I knew all of the major roads, where they led, where the shortcuts were, and how to travel to most locations without ever looking at the pipboy map. It really felt like I had "walked the Mojave" by the end of that playthrough.
