As far as co-op, I don't think it would all that big of a change to the game if they tried to do something like Fable 2 had attempted. Like others have said, you already can get one companion to follow you around - wouldn't really make that big of a difference design-wise whether or not that second character was controlled by another player or the computer. (Actually, if it was another player, they might be able to keep up with me a lot better...)
It's not really my cup of tea, though - I'm not all that big on co-op/multi-player. Just never got into it that much, for most games. But so long as any work put into adding a co-op feature into a Fallout 4 didn't take away from the quality of the single-player experience (ie, if they hired extra resources to work on the co-op mode instead of splitting the workload with the existing team,) then I don't see any compelling reason why I would push to deny other players the option. (And given - it would likely take some real work to get it polished and working, not the least of which is the problem of how you deal with VATS...)
As far as any sort of extensive multi-player beyond co-op, there's already a Fallout MMO in the works. There might be room in the Fallout universe for a more limited multi-player experience, but that could just as easily be another spin-off as opposed to anything added to Fallout 4. (No reason to cram every type of gameplay imagineable into the existing series - that's what spin-offs are for, after all.)
That said, chances seem slim. Bethesda has been pretty consistent in stating that they prefer to focus solely on the single-player experience, and that there are plenty of multi-player games already out there if that's what people want.
