Blue screen, monitor help?

I bought NV for PC awhile back and my PC started acting up. It kept crashing giving me that blue screen only when I played NV. So I sent it in to get fixed twice, and then I wiped everything off my computer and re-installed NV. It worked like a charm for a couple of weeks, now its happening again. So cleared out the game cache and stuff and haven't had a blue screen crash since; but now when I play NV, my monitor just randomly goes to sleep and I can't turn it back on becuase it has 'no DVI signal'.

The thing is, it only happens when I'm playing New Vegas (only game installed since wiping off everything on my computer). I've run scans, file cleanups and tune ups but it didn't dectect anything serious and fixed possible threats.

My graphics card is an ATI Radeon HD 5570 with ultra high settings (deafault) and on Windwos 7.
