I'm pretty sure this issue may relate to DarN's UI but I'm not certain. I've had it happen on and off many times, and the only way to resolve it seems to be begrudging acceptance that I can't use a bashed patch. I have to use a merged patch through FO3edit instead (not necessarily a bad thing, I just prefer wrye bash),

Basically, I...
1) rebuild my bashed patch,
2) activate the .esp,
3) start the game and....
No mouse or keyboard input. It just flicks through the opening reels with the guy in power armour and the Bethesda logo. If I Ctrl + Alt +Delete it'll give an illegal operation.

Like I say, the only way to resolve it is not using wrye bash' bashed patch.

I've seen many answers over the last few years but never a definitive one. Does anyone know what causes this?
