Think about it this way. Do you really think a ghoul could live to be hundreds of years old by being dumb, slow, and weak. It would make sense that most ghouls would survive off of cunning, seeing as they're supposed to be frail.
They are not supposed to be frail; they are in a constant state of regeneration ~that's why they live so long. Ghouls are stiff and awkard, but they hit like a hammer, and in previous Fallout's (where they originated), the Ghouls tended to have more hitpoints than regular humans. Lenny (joinable doctor NPC has an endurance of 6, and starts with 129 hitpoints, where a typical human has between 20 and 40 hitpoints).

The typical human PC started out with 30 hitpoints.

The thing is, having multiple races in Fallout have major impact on storyline. Super Mutants could NOT fit in any storyline, because most people will kill on sight any Super Mutant they see. Ghouls is an iffy, it really depends on the storyline. A ghoul could have worked in NV for the most part, FO3 it was mentioned in the Underworld that BOS kill on sight any ghoul they see, so it couldn't work for that storyline. I mean, I'm not saying it's a bad idea, it would be cool to experience what it is like to be a ghoul or a super mutant, but it doesn't work out for the game. IF they added Super Mutant, the storyline would have to be based around Super Mutants.
