Basically my idea is to implement a wound system, it would work in the sense that certain enemies could cause you to not to beable to recover your full hp.Say you have 50 medicine skill and receive a wound damage and you can only get 50% of your max hp (unless you rest or have a first aid kit/super stim)
You could have a perk called wound licker at 70 medicine that would make you immune.
My idea was that only high level enimies could inflict wounds or if you wear apperal under 5 dr/dt.
