Skill-based accuracy (including ironsights)

I have read through the GECK wiki articles but I still do not understand how to achieve the kind of gameplay I want. I want gun skill to affect only accuracy and not damage and to have it affect both the player and the npcs and to have it work when using ironsights. It should also be really hard to hit your target at low levels and spot on at higher levels. I had hoped that after all this time, someone had managed to do this but since that doesn't seem to be the case I had to dive into the GECK again.

This is what I've done so far:
1) set fDamageSkillMult to and fDamageWeaponMult to 2 - hopefully this means skill doesnt affect damage and damage across all weapons is increased
2) increased fGunSpreadSkillBase and fGunSpreadSkillMult to values ranging from a little more than the default to more than 50x the default. This doesn't seem to do anything and even with 0 skill I can pretty much hit what I aim at when using ironsights.

It's the only thing keeping me from playing NV again as I don't want to go back to having low damage but good aim at low skill levels. When NW was first released, I made a mod for my own use that did something like this through perks but it would stop working after crouching a few times and for the life of me I can't remember how I did it.
