I've got the usual .ini tweaks remove mouse smoothing, and I turned off vsync, however my mouse movement is still nowhere near what it should be naturally. Side to side movement is very slow. I did up my FOV to 90, I'm wondering if I have to change a setting to adjust for that?

You've tried adjusting Look Sensitivity in Settings ?
